I'vre joined a couple of cruises with MSC in Genoa and the flights have always been to Milan. they have arrangements with BA and the cost of the flights to Milan are considerably for them than to Genoa. They will let you buy the cruise on its own and let you organise your own flights but they discount for doing this often doesn't cover the cost of the flights. The coach transfer from Milan malpensa airport takes about 2 and a half hours. An alternative to the coach, if you are doing your own flights, is the train. Does take almost as long as the coach transfer but is a lot more pleasant journey. the trains are very comfortable, cheap and punctua.
