Yes you can. Lots of people do it, on my last cruise 4 people tied the knot and 7 renewed vows.

Only on ships that offer a marrying package - such as on some of the larger Princess ship.

Yes but you need to make the appropriate arrangements in advance many couples renew their vows and that is popular. Some ships have their own little chapel for these services or they use a private room.
